Who this form is for

This form is for current DPZ Kootenays employees who want their one-time DPZK Team sign-in code sent by text message instead of to their work email. It is not a sign-up for customers, and it is not a marketing list.

Give your consent

What we record, and why

When you submit this form we record the mobile number and work email you entered, the date and time, the exact wording of the consent you agreed to, and the IP address the form was sent from. We keep that record as evidence that you agreed, and we show it to our messaging provider if they ask how consent was obtained. It is handled under our Privacy Policy.

Stopping the messages

Once text-message sign-in is switched on, reply STOP to any message to stop receiving them, or HELP for help. You can also ask your manager or area supervisor to remove your number. Stopping the texts does not affect your work email sign-in.
